Graphics Request Form

This form has been developed by External Relations (ER) to help you plan and meet your goals for any visual or printed materials that are not exhibition signage. Please allow AT LEAST TWO WEEKS FROM RECEIVED WORK REQUEST to final design layout (not including print time) as a minimum timing outline when planning your project. Please fill out all requested information.

You are responsible for any materials costs required by your project, such as copying, printing, or photography. These are separate charges.

You are responsible for final proofreading and getting any necessary approvals for the project.
